National Health Center Week Celebration

National Health Center Week has been celebrated for more than 30 years to recognize the services and contributions of Community, Migrant, Homeless and Public Housing Health Centers. While there are countless reasons to celebrate America’s Health Centers, among the most important and unique is their long success in providing access to affordable, high-quality, cost-effective health care to medically vulnerable and underserved people throughout the United States. Here at Hamilton Community Health Network we provide a multitude of community events throughout the second week of August as an opportunity to give back.

Outreach & Enrollment Specialists

We have a dedicated team of Navigators and Certified Application Counselors (CACs) to assist at each of our locations with enrollment in the Affordable Care Act health insurance plans of your choosing. We will answer any questions you may have, and assist if you need to switch plans or file an appeal.

Open enrollment begins November 1 and runs through January 31st of each year. Call to make an appointment for one of our assistors to sit down with you one on one at (810) 787-5097.

Homeless Outreach Program

Our Homeless Outreach Program was created to promote health, hope, and dignity for the homeless population in our service areas. We provide access to comprehensive services at all of our conveniently located clinics. Many of our services are free to those who are homeless and in need. We partner with various agencies throughout Genesee, Lapeer and the surrounding areas.

Through collaborations and partnerships, we provide needed services to hundreds of homeless individuals and families each year. By working together, we can continue to provide hope, health, and dignity to the homeless population in our community. Each year, we provide services during Project Homeless Connect events in Genesee and Lapeer Counties.
