We accept the following payer sources:
- Most commercial insurance plans.
We also welcome patients with no insurance. Hamilton Community Health Network is NOT a free clinic, but if you are uninsured or under-insured we offer a few ways that we may assist you.
If you have any questions, please call 810-406-4942.
Hamilton Community Health Network has Outreach and Enrollment staff trained as Certified Application Counselors (CACs). Our CACs assist with Medicaid plans such as the Healthy Michigan Plan and MI-Child or Healthy Kids, the Genesee Health Plan, and health plans offered on the Marketplace through the Affordable Care Act.
To find out more about insurance enrollment assistance, visit our Health Insurance Enrollment page.
In addition, Hamilton Community Health Network has a DHS caseworker to assist you with information on medical coverage, as well as state services, such as food or cash assistance.
If you do not have insurance, please call the Enrollment Hotline at 810-787-5097. Please leave a message with your name and phone number and someone from our Enrollment staff will return your call to set up an appointment.
Prescription Assistant Workers (PAWs) are located at Hamilton’s North Pointe, Main and Burton clinics. The PAW assists patients in need who can not afford their prescription medication. The PAW determines eligibility for appropriate prescription programs and assist patients in accessing medications.
Patients who receive medical treatment without health insurance may be eligibility for discounts on services. The Sliding Fee is based on the patient’s ability to pay and is determined by the individual’s and family’s annual income. Fees start at $10 for medical services, and $25 for specialty services such as dental, vision, podiatry, behavioral and more. For more information, give us a call at 810-406-4246.
