Anyone can be seen at Hamilton regardless of your ability to pay, insurance, or citizenship status. If you do no have insurance, we have a Sliding Fee Scale to help you pay for your medical services.

Health Insurance Marketplace – Open Enrollment

We have a dedicated team of Certified Application Counselors (CACs) to assist with enrollment in the Affordable Care Act health insurance plans of your choosing.
We will answer any questions you may have and assist if you need to switch plans, or file an appeal.
Marketplace Open Enrollment runs from November 1 through December 15.

Hamilton Community Health Network is part of a national network of primary care centers (Section 330E) aimed at improving health services for inner city populations. These centers provide comprehensive care to patients regardless of their ability to pay. Hamilton puts patients first, with its caring, trained, certified staff that is dedicated to the management of chronic and acute disease, oral health management, health promotion and disease prevention.
